Welcome to my practice!

 

The Nature of my Practice

 

In my healing and counseling practice, I offer various services for my clients to choose from. I offer core shamanic work addressing healing and divination in person or long-distance, Barbara Ann Brennan hands-on-healing work in person or long-distance, and QHHT Past-life Regression Hypnosis sessions following the Dolores Cannon method - ONLY in person. Additionally, I offer general mentoring and counseling services via video conferencing, over the phone, or in person.

I am not a licensed physician, and my services are not medical treatments, nor are they licensed by the state of California.

 

I may provide you with spiritually-based services of energy healing, core shamanic practices, QHHT Hypnosis, and spiritually based counseling and mentoring.

In order to use my services, California state law requires that you acknowledge receipt of the information provided in this form and that you sign it. You will receive a copy and I will keep a copy in my records for at least three years.

The services offered by me are alternative or complementary to healing arts that are listed by the State of California under sections 2053.5 and 2053.6 of California’s Business and Professions Code. I can offer you these services, subject to requirements and restrooms that are described fully in the document called: California Senate Bill SB-577, What it Means for Patients. You will also receive a copy of this document.

If you ever have any concerns about the nature of your treatment, please feel free to discuss them with me. I recommend that you inform your medical doctor or other professional health care provider you are currently receiving services in any of the former listed above.

I have read and understood the above disclosure and discussed the nature of the services to be provided. I understand that Sabine Schmidt is not a licensed physician and the services offered are not licensed by the state. I understand it is my responsibility to maintain a relationship for myself/my child (under 18 years of age) with a medical doctor.
